CMS provides HCRIS source data files and documentation for the 2011 and 1994 version of the files. The Provider Reimbursement Manual versions 265-11 and 265-94 files have an instruction manual and worksheets.

This 265-11 and 265-94 worksheet materials are helpful for identifying the worksheet, line, and columns of the variables.

Select variables from the cost reports is available in the files below. One of the variables included is ZIP Code. A ZIP Code distance database is available.

NBER affiliates can access this and other data through the NBER computing system.  Files can be located using the directory path of each download link replacing "https://data.nber.org/" with  "/homes/data/".
